Insurance & Payments

At Great Divide Physical Therapy, we accept most insurance and are preferred providers for many companies. In order to bill your insurance and avoid any issues, we will need your current insurance information including your claim number or insurance card before your first appointment.

We will call your insurance company to check your benefits and will let you know about any co-pay or expected out of pocket cost in order to allow you to plan and adjust your care to meet your needs.

Please note, the information given to us by your insurance company is an estimate. Exact amounts are not determined until the charges are billed through your insurance company and the amount of time this process takes can vary. We will do our best to ensure that treatments are billed from our clinic in a timely manner, but issues submitting claims and processing these claims can still cause delays.

  • VA (Government Plans)
  • Referrals Or Prescriptions

Does My Doctor Need to Refer Me for Treatment?

You do not need a physician referral to be treated with physical therapy in the state of Montana, however, some insurance providers require a prescription or signed plan of care in order to receive a reimbursement.  We recommend you contact your insurance company or plan to determine if you need a prescription or authorization for them to cover care.

Any questions you have regarding what your insurance provider requires can be answered by calling the customer service number for your plan.

Fee-for-Service

If you have a high deductible insurance plan, no insurance, or would prefer to not bill insurance for your treatment, Great Divide Physical Therapy offers an alternative. Our Fee-for-Service is a pay-as-you-go, flat-rate system that allows you to make all the decisions regarding your treatment and what is right for you. The costs are less than the typical private insurance billing. At the end of each visit you will pay for your treatment and receive an itemized receipt. Great Divide Physical Therapy will not be involved in any insurance billing process.

Fee-for-Service Rates:

Initial Evaluation: $160
Additional Treatment: $125 for each 40 minute appointment, $160 for each 60 minute appointment
Individual Modality Treatment (e.g., ultrasound): $65 per treatment
